When to Replace Your Toothbrush (and What Happens If You Don't)

A worn toothbrush cleans less than you think. Here's how often to swap it, what to watch for, and the surprising reasons to replace early.
Most people keep a toothbrush longer than they should. The standard recommendation — replace every 3–4 months — is short for a reason. Here's what changes as a brush ages and when to make the swap sooner.
Why 3–4 months is the sweet spot
Even with careful use, bristles start to bend, splay, and lose their spring within a few months. Once the tips are no longer straight and rounded:
- They can't flex into the small grooves between teeth
- They slip past the gumline instead of cleaning along it
- They're rougher on gum tissue, which can cause irritation
One study found that a brush older than three months removes noticeably less plaque than a fresh one — even if it looks 'fine' to the naked eye.
The five signs it's time to replace early
- Splayed or bent bristles. The number one signal, and the number one sign you may also be brushing too hard.
- Dark spots or discolouration. Mould or mildew from a brush that never fully dries.
- After a cold, flu, or strep infection. Bacteria and viruses can linger on bristles.
- A lingering odour that rinsing doesn't fix.
- After dental work where you were told to keep the area extra clean, like a new crown or implant.
Electric toothbrush heads
Same rule: every 3–4 months. Many premium heads have blue indicator bristles that fade to white — a helpful visual cue, but not a substitute for checking the shape.
How to make a brush last its full life
- Rinse thoroughly after each use to clear toothpaste and debris
- Store upright so it can air-dry between uses
- Skip the closed travel cap at home — trapped moisture breeds bacteria
- Keep brushes apart so heads don't touch
- Lighten your grip — pressing hard shortens brush life dramatically
