The New-Patient Exam: What Your Dentist Is Actually Checking For

A behind-the-scenes look at every check your dentist runs during a new-patient exam — teeth, gums, bite, and oral cancer screening.

A new-patient exam looks like a quick peek with a mirror, but a lot is happening in those few minutes. Here's each layer we're assessing and why it matters.

1. Soft-tissue and oral cancer screening

Before we look at teeth, we check your lips, tongue, cheeks, floor of the mouth, and the roof of your palate for colour changes, lumps, or sores. Early-stage oral cancers are highly treatable — but only if someone's looking. We also feel the glands along your jaw and neck.

2. Gum (periodontal) assessment

Using a thin probe, we measure the small pocket between each tooth and the gum. Healthy pockets sit at 1–3 mm. Deeper readings, bleeding, or recession tell us whether you're dealing with gingivitis, early periodontitis, or something more advanced.

3. Tooth-by-tooth check

Each tooth is inspected for:

  • New or recurrent decay (including under old fillings)
  • Cracks, chips, and wear facets from grinding
  • Loose or failing restorations

4. Bite and jaw joint (TMJ) check

We watch how your teeth come together, listen for clicking at the jaw joint, and look for signs of clenching — flat spots on molars, scalloped tongue edges, or morning jaw soreness.

5. Existing dental work

Crowns, bridges, implants, and fillings all have a lifespan. We note the age and condition of each so there are no surprises later. If a crown is due for replacement, same-day options exist — see our CEREC crowns guide.

6. Risk scoring and recall interval

Finally, we score your decay and gum-disease risk. Low-risk patients may only need cleanings every 9–12 months; higher-risk patients benefit from 3–4 month intervals. Your recall schedule isn't one-size-fits-all.

A written summary of findings, a prioritized treatment plan, and clear cost estimates with insurance applied. Nothing starts without your go-ahead.
