Bone Grafting for Dental Implants: When It's Needed and What to Expect

When a tooth has been missing for a while, the jawbone in that spot shrinks — sometimes enough that an implant needs a stronger foundation before it can be placed. That's where a bone graft comes in.

Why bone volume matters for implants

An implant needs bone on all sides to integrate securely. If the ridge has thinned or the sinus above an upper molar has expanded downward into the space, there simply isn't enough bone to anchor a standard implant. A graft rebuilds that volume so placement becomes predictable.

Common situations where grafting is recommended

• The tooth has been missing for a year or more and the ridge has narrowed
• A tooth was recently extracted and the socket needs preserving
• An upper back tooth site needs a sinus lift to gain vertical bone
• Gum disease has eroded bone around the site

What the procedure involves

Graft material — which can be your own bone, a processed donor material, or a synthetic — is placed at the site and covered with a protective membrane. Over three to six months, your body remodels the graft into your own natural bone. In some cases, a small graft can be done at the same visit as implant placement; larger grafts are usually done first and healed separately.

Recovery in plain language

Most patients describe the discomfort as similar to an extraction. Expect some swelling for two or three days, a soft-food diet for about a week, and careful rinsing rather than vigorous brushing at the site. Sedation is available if the idea of the procedure makes you tense.
